Volunteer Interest Form

Team Manager's are responsible for identifying certified volunteers to help throughout the season. Volunteers can include:

• Locker Room Monitors*
• Dryland Monitors*
• Penalty Box Volunteers*
• Equipment Managers
• Videographer/Photographer
• Event Coordinator
• Statistician

*Requires USA Hockey Number, Safe Sport & Background Check

If you would like to volunteer, please complete this form, the registrar will then be able to add them to your team’s contact sheet. A Team Manager is unable to enlist help from volunteers until they are approved by the registrar.
